He's not doing well.	She's doing well.
Frank is not doing well. He is still smoking. He is going to have another operation tomorrow.	Marie is doing well. The nurse said she can probably go home from hospital tomorrow.

HOW ARE YOU? (someone who is not sick)
I'm really good.  (100%)
I'm pretty good. (70% - 100%)
I'm good.           (50% - 100%)
I'm not so good. (20% - 50%)
